D: 17 Concierge affiliate program

 How it works:
A Percentage of the sale, that would be set to be paid to the sales person, is instead, paid as a commission, under a 1099 structure, to the Affiliate refers.
To Qualify; the following data must be submitted by you:
Your affiliate number (issued by D17)
Your email address
Primary Passenger First and Last Name
Number of passengers
Primary Passenger email address
Primary Passenger Cellphone number
Day and date of travel
Type of Ground transfer needed:
-Point to Point (P2P)
-As Directed (3 hour min) (AD)
-If incoming flight
-Airport Landing/Airline flight number
-Or FBO Landing
-Eta
-Destination
Any special accommodations/requests?

The referral must book, and Commission will be paid at the conclusion of the service, after completed in it's entirety.  
A Check will be mailed automatically to the mailing address on file.

Once the form is submitted, inform the guest/passenger that someone from the executive lift group will process the request, and get a quote out to you, as soon as possible.
